1. Patients admitted with Diabetes mellitus with renal insufficiency and collected data such as blood levels and vital functions.
2. Some of the patients when they have been re-admitted to the hospital after 6 months to 1 year because of complications of lacking monitor or information from primary healthcare.
3. They have given the questionaries when they have second-time admission and make the comparison 1st and 2nd admissions with the blood levels such as glucose levels, HbA1c levels and kidney function levels.
4. Results should be needed to improve the performance of the primary healthcare sector.
Totally 100 patients have been collected for this dissertation work and the age limit is 40-80 years old.
In first admission, 100 patients have chosen and treated
After 6months period readmission of 30 patients have been re-admitted to the hospital because of complications and worsening of kidney function and elevated glucose levels.
After 1 year totally of 67 patients have been readmitted to the hospital because of complications.
In conclusion, it should be saying 67 out of 100 patients have been re-admitted to the hospital because of complications and needed to improve the primary healthcare levels.
