Start with an outline of Problems/Issues to address.
Using the SMART format, develop three service goals for one issue identified.
Now outline your full-service contract including the action steps that must be taken underneath each goal. Remember to keep them SMART! Include at least
two action steps for each goal. You can identify client and worker tasks for your action steps. Make sure your action steps relate to their respective goal. For
example:
Goal 1: Client will gain full-time employment within 6 months of today’s date.
Client will review job advertisements at least for 30 minutes daily.
Client will contact a temporary within two weeks.
Client will submit at least five applications for full-time employment a month.
Worker will identify a contact person with the Office of Vocational Rehabilitation for client referral by time of next session in one week.
Evaluate your contract in terms of its relevance to this client and the depth of which it covers the issues needed for this client. In 2 – 4 paragraphs if not in
question-and-answer format, please address the following:
Relevance:
How does your contract relate to the client’s problems?
How does your contract help to resolve the client’s problems or issues?
Depth:
1. What makes this a difficult contract to develop?
2. What are some of the complexities of the case?
3. What are some of the difficulties or obstacles you might have to deal with to implement this plan?
